Club Advisory Committee

All current puppy raisers and co-raisers should have access to the online forms page of the Guide Dogs for the Blind, Inc. website.  Go to www.guidedogs.com -> Programs -> Dog Programs -> Puppy Raising.  On the right side of the page should be a box entitled Puppy Raising Forms & a link “Go to all puppy raising forms.”  If you do not have access to these forms, please e-mail Sarah Netoff at snetoff@guidedogs.com to have your online account updated.

There is lots of good, current information on the GDB forms page.  Below are links to the basics.

Puppy Raising Manual

All raisers are required to have a current Guide Dog Puppy Raising Manual.  Click on these links to download and print copies of the manual and appropriate supplements:

Monthly Progress Report

A written progress report on your puppy must be submitted each month to the club leaders, Warren & Claudia Wish.  They review your report, add their observations, & forward it to our club’s Community Field Representative from GDB.  Following her review and follow up (if necessary), the report is filed in the puppy’s permanent record to be read by the GDB instructors during your puppy’s formal training.  These reports are very important and should be kept up to date.  Although you can download, print and complete the monthly report by hand, it is easier for all concerned if you complete the report online & forward it by e-mail to the club leaders.

Project Record

When it is time for your puppy to return to GDB for his formal training, you will be asked to complete his Project Record.  You received a hard copy of this document in his puppy packet.  As with the monthly progress report, it simplifies the process if you complete the Project Record online and e-mail it to the club leaders.  It is very important this document be completed at least one month prior to your puppy’s return to GDB.  The club leaders and our GDB Community Field Representative must have time to add their observations and comments and forward it to the Training Dept. at GDB before your puppy arrives.

#8 - Puppy Project Record (Word) - This Word version of the Project Record will allow you to save it to your own computer; you can type in content, save, and return to the file on your computer as often as you choose, and print it out when ready.

#9 - Puppy Project Record (PDF, 40KB) - This PDF will allow you to print out the Project Record and complete the form by hand.

#10 - Puppy Project Record (PDF, 44KB) - This PDF will allow you to complete the Project Record on your computer screen and print it, but it cannot save the data. Only use this form if you're ready to complete the Project Record in one sitting and do not need to save or e-mail it. (Recommended only for those who do not have access to Word.)

 

Fun Things About My Puppy

This form must be completed and given to the kennel staff when you return your puppy to Guide Dogs.  When your puppy is matched with his new person, they are given a copy of this to learn all the special things about your puppy that only you, the raiser, can provide!
